Dr. Edward Formica
My goal is to provide you and your family with exceptional dental care, in a fun, comfortable environment.
I have been practicing Dentistry in Hemet since I graduated from UCLA in 1990. I am a member of Tri-County Dental Society, American Dental Association, California Dental Association, Hemet/San Jacinto Exchange Club, Healthy Valley Foundation, Valley Trails Alliance and the Western Science Board. We have been working for years with valley charities and education, including Valley Wide Recreation, valley schools, boy scouts, Ramona Bowl, Western Center and many individual athletic and student groups. Our efforts helped build the aquatic center, Diamond Valley recreation center, Searl Park, Bill Gray park exercise trail and the upcoming Jim Venable Exchange Club park in Valle Vista.
My wife Chris & I have lived in the Hemet area since 1990. We are active in both this community and our church. We have enjoyed raising our 4 children in this valley. We hope the next generation has the same opportunities.

Jane Shepherd R.D.H. (Registered Dental Hygienist)
Hi! My name is Jane Shepherd. I’ve been married to my wonderful husband, Tom, for 37 years. We have four awesome children and two pet dogs. Through the years we have enjoyed travelling the U.S. and abroad, both together and as a family. One of our goals is to visit as many of our National Parks and Monuments as possible.
I was born and raised outside of Buffalo, New York a long time ago! It’s a great place to be from, and where my husband and I will retire, except in the winter months. We have brothers, sisters, a son, and many other family members and friends in that area, so we visit Western New York often.
I’ve been a dental hygienist for 30+ years. After moving to California, I worked and lived in Torrance, Laguna Hills, Rancho Santa Margarita and now live in Temecula and work in Hemet.
Working for Dr. Formica the past 8+ years has been a blessing. Our dental office is professional, comfortable, and friendly. It is a great place to work and I know I will have long lasting friendships with my fellow staff.
Dianna Lopez – Patient Care Coordinator
Hello! I have been in the dental field for over 20 years. I am originally from Northern California and have lived in Hemet for 2 years. After graduating from Modesto High, I decided that I would like to work in the dental field, so I attended Galen College to become a dental assistant.
As I worked, I gained knowledge and experience. I am passionate about my job and making sure our patients are satisfied and happy with their smiles.
Working for Dr. Formica a is a blessing. Our office is family orientated and professional. My fellow co workers and I work together as an excellent team!
Krista Macintosh R.D.A. (Registered Dental Assistant)
Krista Macintosh has been a Registered Dental Assistant Since 2011; she joined our team at Formica Family Dentistry in 2015. Krista has five children ranging in ages from 9 to 21. One of her favorite things is to watch them play baseball and softball. She also likes to take them camping and hiking.
In her spare time, she enjoys baking. (Opinion by Doc. “Krista is being modest here, she is a talented cake decorator, she is creative and passionate about baking from scratch.”)
Krista says the thing she enjoys the most is dentistry, especially extractions. (Is that weird?) We have enjoyed having Krista with us for the last 3 years; she is hardworking and kind which is a win-win for our patients.
Louella Giles R.D.A. (Registered Dental Assistant)
I have been working in dentistry for 10 years, the first 3 years were volunteering for the Red Cross in Italy. Before I began work for Dr. Formica, I worked in Murrieta assisting with general and implant dentistry.
My husband is a retired Navy veteran and together we have 2 daughters, the oldest currently goes to San Francisco State University and the youngest is still in high school.
I was born in the Philippines and raised in Japan. I met my husband in Okinawa. Our family moved to the United States after the birth of our second daughter.
We like to travel all over the world and experience many new things and sights, including my favorites, Munich Germany and the Eiffel Tower in Paris.
I am coming up on my third year here with Doctor Formica. I look forward to helping our patients with their dental health.
